Receipts for each visit will be provided for you at your request to submit to your insurance carrier. Some insurance companies and all HMO's require co-payments at the time of service. Many traditional plans, including Medicare, require you to meet a deductible every year.

Listed below are the major health insurance companies with which we are enrolled providers, along with their websites and phone numbers. Some behavioral health benefits are managed by specific companies, which are noted below. If your health plan is not listed, please contact us. Because each health insurance plan varies, we recommend calling the phone number located on the back of your insurance identification card to verify your mental health and substance abuse benefits. It is a direct link to professionals who can give you immediate, confidential assistance. You may also wish to speak with your company's Human Resource Department for more information regarding your insurance plan.
